HealthEmove wants the best possible care for people often changing healthcare providers

Our mission is to give health care workers access to the medical record of a ‘person on the move’ visiting their clinic for care. With insight into the medical history of their patients, healthcare workers can make the best decisions.

Patients can decide who has access to their record. Both the patient and the healthcare provider can add data to the medical record, but you can see who added the data.

We create HealthEmove in co-creation with vulnerable groups in Amsterdam and their healthcare providers. We intend to designate certain organizations and their health care workers to edit the information in the person’s file so that the information is up to date and gives a quick and useful overview for other health care workers who have not had the chance to work with the app.

How does it work?

We are currently working on a healthcare provider portal. 

Since this is not yet in place please follow the next steps: 

  1. Ask a patient if he/she already has a HealthEmove account. If so, ask for insights into their previous medical documents. If not, promote the use of HealthEmove.
  2. Allow a patient to take a picture of their medical record. Print the record or allow a picture of your computer screen. 
  3. Help the patient add the correct medical information to HealthEmove.

As healthcare providers we must work together to provide the best possible care for people on the move and eventually safe time.

To avoid double documentation, we ask patients to take photos of referral letters, prescriptions and pictures of the screen of healthcare providers.
